This is a “fillable” form.  There are multiple ways for you to submit this form.  (1) You can download the form, fill it out, and then email it to infinite.campus@caldwell.kyschools.us. OR (2) You can download the form, fill it out, print it and then bring it to the office of Laura Lee White at the Caldwell County Board of Education (612 W. Washington).  Once the form is submitted, it will be quickly reviewed for acceptance.  You will receive a phone call or an email as soon as you are cleared to enroll your child through our online registration system.
